Senior Trip Information

 

Below are a list of important reminders and forms for students and parents to review prior to the trip.


Meetings

Each student, along with a parent/guardian, MUST ATTEND one of the two mandatory meetings held prior to the trip.

Required Forms

Each student MUST SUBMIT a Parental Consent Form, and Emergency Information Form along with a copy of your health insurance card. We suggest you make a copy of the completed forms for your own records.

Medication

Any student planning to bring any kind of medication (prescription or over the counter) must fill out a Class Trip Medication Form with the nurse’s office (Ms. Gillespie or Ms. Varga). All questions about medications will be answered at the meetings prior to the trip.
