Student Registration

• Children are eligible for kindergarten in the 2013-2014 school year if they will be five years of age on or before October 1, 2013.

• Prospective kindergarten students MUST HAVE AN APPOINTMENT TO REGISTER.

• Appointments can be made by contacting the Maple Grove Administration Building at 629-6400 x1003.

• The following forms are required for kindergarten registration:
  • Kindergarten Pre-Registration Form
  • Student Data Entry Form (PDF)
  • Admission Physical Form
  • Affidavit of Residency
  • Emergency Card*
  • Acceptable Use Packet (Elementary Students)
  • Transportation Forms

*In addition to local media notifications, emergency information is delivered by an automated notification system called Global Connect which provides real-time information regarding school closings, late openings, early dismissals and other emergent notifications. The system is occassionally used to announce school-related activities and special events. If an automated message is distributed via Global Connect the following phone numbers will be contacted: Home Phone, Mother's Day Phone, Mother's Cell Phone, Father's Day Phone, Father's Cell Phone, Designiated Global Connect Phone. Global Connect will only dial a ten digit phone number, it cannot dial an extension number.

• Permanent school record files are created for each prospective kindergarten student.

• Kindergarten teachers are available for questions and concerns.

• Speech professionals are available for speech/language concerns.

• STA will provide bus safety materials and give the children and opportunity to board a school bus.

• Registration will take approximately one hour to complete.

• You do not need to bring your child.

• Your child's original birth certificate

  It must have a raised seal;  Hospital issued documents are NOT accepted.

• Immunization (shot) records. 

• Proof of residency. 

  Property tax bill, settlement papers, lease, rent agreement or notarized landlord affidavit with property tax bill

• Legal guardianship papers, if applicable

• Admission physical form

  To be completed by your child's physician and returned to the school prior to start of school in September

• Your child's original birth certificate

• Specific information regarding Kindergarten orientation will be available at registration.  The orientation will feature an overview of the upcoming kindergarten programs with various group activities involving the children.  In order to participate in the orientation, registration must be totally completed.
