Enrollment Documents
Before You Begin
Having your documents ready before you start will help you complete the registration process efficiently.
Please review the required documentation checklist below and ensure all applicable records are ready to upload.
*Note: Current Monroe Pre-K students will automatically enter Kindergarten for 2026–2027

(Monroe Township / Williamstown, Gloucester County) Provide any ONE of the following:
- Current property tax statement
- Signed lease
- Signed settlement sheet or deed, if you've closed on your home within the last 6 months
Not the property owner or leaseholder? Submit a notarized Affidavit of Residency Form along with a signed lease or property tax statement.

- Student's state-issued birth certificate (hospital certificates are not accepted) — or a passport if the student was not born in the U.S.
- Parent/Guardian photo ID
- Proof of legal guardianship or court orders, if applicable

New Jersey law (N.J.A.C. 6A:16-2.2) requires every student to have a physical exam upon admission, completed within 365 days of their start date.
A note from your doctor isn't enough on its own — it needs to be the official physical exam form. — Questions? Contact your school nurse.
